Insurance & Payment Disclosure:
In an effort to offer transparency in coverage, each service has a good faith estimate for self-pay individuals. Rates are based on the sliding fee scale being applied.
We will submit insurance claims on your behalf unless you elect to self-pay. Copayments are due at the time of service and all patient-responsible balances, including deductibles, coinsurance, services not covered by insurance, and uninsured cash payments are due on the date the balance is posted. A valid credit, debit, or HSA card is required to be placed on file for all commercial plans and patient responsibilities. Unpaid balances will be automatically charged to the card on file after the balance populates, typically following insurance processing.
We offer therapy with medication management, typically involving longer and more frequent appointments. Private Pay ranges from $201 to $352. These claims are also sent to insurance for payment.
Medication Management check-ups are brief (20-40 minutes), with fees billed to your insurance. Private Pay is on a sliding scale: $118 - $252 for follow ups or $201 to $235 for a first appointment (up to an hour).
Everyone heals in their own way, and for some, holistic treatments are part of that process. All holistic appointments include evaluation and management and are billed to insurance. Private pay ranges from $100-$300.
Therapy appointments are scheduled for 1 hour, typically on a weekly, ongoing basis. All services are billed to insurance. Private pay rates are $170 for the initial intake and $165 for thereafter. If you and your therapist agree that shorter sessions are a better fit, private pay rates start at $70.
Weight has always been a mental health concern. At Mending Mental Health, our certified providers coach those who might qualify for this treatment and decide which one is the best option for each person. Compounded injections are provided on a monthly basis with all the tools you need to lose weight and keep it off. Cost Varies.
Our comprehensive testing can be performed virtually and is billed through insurance whenever possible. Your insurance provider will process the claim based on your specific plan and benefits. For those paying out of pocket, the private pay rate is $1,500. This includes a detailed evaluation report and referrals for follow up care, perfect if you’re simply seeking clarity and next steps, without committing to ongoing services.

Genetic testing is easily ordered by one of our providers and billed directly through the lab. You might receive an Explanation of Benefits (EOB) in the mail, but don’t worry, it’s not a bill. Most patients pay little to nothing out of pocket, thanks to a sliding fee scale determined by the lab itself.
